EMERGENCY FOOD AND SHELTER PROGRAM (EFSP) FUNDING
HOW TO APPLY FOR PHASE 28 (2010) FUNDS
APPLICANTS NOT FUNDED IN 2009 ARE REQUIRED, AND ALL APPLICANTS ARE ENCOURAGED TO BE REPRESENTED AT THE PHASE 28 APPLICANTS WORKSHOP, TUESDAY, JANUARY 26, 2010, 2:30 5:00 PM, AT 3315 AIRWAY DRIVE, SANTA ROSA (entry faces Center Street).
Non-profit or government agencies that currently provide emergency food and/or shelter in Sonoma County and meet the criteria in the Request For Proposals may apply for EFSP funds, which originate from the Federal Emergency Management Agency (FEMA), to expand these services.
You may either:
1.) Download the proposal documents below, starting with the Request for Proposals to determine eligibility and funding priorities. Review the Cost Chart for eligible expenses. Complete the EFSP Phase 28 Application form on your computer, answering each question in order, include a program budget formatted as requested (See Sample Budget Format), and print and copy the completed application for timely submission (See Phase 28 Timeline). This is the recommended option.

2.) Download and print the documents below including the EFSP Phase 28 Application form, OR pick up printed Application packets at the Sonoma County Task Force for the Homeless, 3315 Airway Drive, Santa Rosa (entrance on Center Street). Please call first, since the office is currently staffed only part time. The office phone is 575-4494. Complete the form by typing in the information, adding supplemental sheets as needed. Please clearly label the section numbers and questions on any supplemental sheets, and attach them in order to the Application, prior to the budget.
PROPOSAL DOCUMENTS TO VIEW/DOWNLOAD:
All Applicants should download and review the most recent EFSP Manual and Key Changes from www.efsp.unitedway.org, the EFSP National Board web site, before submitting an application. Please especially note the detailed requirements for documenting EFSP expenses. The Phase 28 Manual and Key Changes are not yet posted on the EFSP National Board site as of the date of this Request for Proposals. You may consult the Phase 27 manual until the new one is posted, since changes are likely to be minimal.
Applicants must submit 12 printed copies of the completed Application form and one copy of each of the supporting documents listed on the Application by 5:00 pm Wednesday February 3rd, 2010 at the Sonoma County Task Force for the Homeless office, 3315 Airway Drive, Santa Rosa.
